The waiver is available online, and links have been placed in the Cohort Google Classrooms (ask yor child to open and sign it from their device. Complete the form and obtain all required signatures.
Return the completed form to the Main Office.
A counselor will retrieve the form and schedule an appointment to determine if the change can be granted.
Students are strongly encouraged to act promptly and ensure all signatures are obtained before submitting the form.
Be ready for the 1st day of school.
